Stopped, not queued
Waiting and blocked look identical on a board and are completely different problems.

Most work tracking systems have one state for work that is not moving, and that single state hides the only distinction that matters. Queued means the work is fine and is waiting its turn. The constraint is capacity. Stopped means the work cannot proceed until somebody decides something. The constraint is a decision, and capacity will not touch it. You can add three people to a stopped item and it will not move. Organisations conflate these constantly, and the consequence is predictable: capacity gets added to solve a decision backlog, and the new capacity produces more stopped work. So name anything stopped, with the decision it waits on and the person who owes it. And look at the top three by name, every review, before anything else.

Stopped means the work cannot proceed until somebody decides something or supplies something. The constraint is a decision, and capacity will not touch it. You can add three people to a stopped item and it will not move.
Organisations conflate them constantly, and the consequence is predictable: capacity gets added to solve what is actually a decision backlog, the new capacity produces more stopped work, and the operation concludes it needs more people again.
The instrument is a rule rather than a report. Anything stopped is named, with the specific decision it is waiting on and the specific person who owes it. Anything stopped for longer than an agreed period goes to arbitration. And the operating review looks at the top three stopped items every time, by name, before it looks at anything else.
Three, not all of them. A list of forty blocked items is read as weather. Three with names against them is read as a decision the room is about to make.
